Forest City Nursing & Rehab Center is a 132 bed facility nestled in the country setting of a small community. We have served the needs of our residents since 1982. Residents and their families are invited to participate in the many activities offered on a daily basis. You can take advantage of the spacious family room, comfortable sitting room, outdoor pavilion and Meditation Garden while enjoying the picturesque scenery that changes with the four seasons. Our goal is to bring individuals to their highest level of functioning while protecting and promoting their rights. Our caring staff and home-like environment provide a warm, compassionate atmosphere for our long-term and short term residents. Residents are encouraged to bring cherished possessions in order to preserve their association with family and friends.
